Here are a few observations from my experience of actually taking the survey. I hope these are helpful. * The drop down boxes with an option for "Other, please specify" offer no chance to specify anything unless a new separate text box is available at the end of the question (as it is in Q2, I think). * On Q4, the drop down options look like they should change with each piece of the question. For example, when responding to the "gender" or "age" or "at risk population" questions, the one-size-fits-all drop down options don't seem to work. * On Q5, does the drop down option "pre-school" make sense? Is this training really ever going to pre- or middle-school aged kids, or might this refer to the faculty that teach at that level? (Sorry if this is obvious and I just missed it). * Qs 6, 7, 8 make me want to also ask them whether they altered the materials in any way for their audience (not just which pieces they used from each kit).
