Each school entity will need to print, complete and return three forms (Participation Summary, Letter of Agency, Form 479). Schools without NSLP data reported to MDOE need to file additional forms as instructed. The Form 470 must be posted for 28 days before you can select a vendor and file the next form, the Form 471. The deadline for Form 470 is January 15, 2009 The deadline for the Form 471 is February 12, 2009. Do your Form 470 today!
