Form 471- Item 21 Attachment Glitch

 

I have had a few calls regarding a problem with the Item 21 Attachment.

If you have just submitted your Form 471, certified it, then clicked on
the Electronic Certification button you may be unable to proceed to
submit the Item 21 Attachment because there is nothing to "click" on
that would allow you to go forward.

 

There is an easy solution.

Go back to the USAC Apply Online page

http://www.sl.universalservice.org/menu.asp

 

Under Form 471, click on the link for Item 21 Attachment 

You will need your entity #, Form 471 Form # and Security Code.

If you have submitted and certified the Form 471 you should be able to
proceed and find now you actually have something to click on and can
proceed.

Follow the sample wording for the Item 21 attachment found at:

http://www.maine.gov/msl/erate/atm/

 

 

If you are unable to complete the Item 21 Attachment following this
"fix" please make sure that you have indeed submitted and certified the
form.

You can do this at the USAC Apply Online page as well.

 

Go to:  http://www.sl.universalservice.org/menu.asp

Select Continue Incomplete, if you can proceed, then you did not submit
your form

Select Certify Complete, if you can proceed, then you did not certify
you form

Select Display, check what it says in the Application Status box.
