The deadline for Certifying the Form 471 is Wednesday, March 12, 2008 Also, make sure you have submitted your Item 21 Attachment as well. To Certify Form 471 online: Go to: http://www.sl.universalservice.org/menu.asp Under Form 471, click the "Certify Complete" Enter your Form 471 application number and your security code. Click the Electronic Certification button (if you have a PIN) Click Paper Certification (if you do not have a PIN) If choosing Paper Certification, make sure that your paper certification is postmarked before or on March 12, 2008. If certifying electronically, it must be done before midnight, Wednesday, March 12, 2008 Display your Form 471 and get a certification date/confirmation Go to: http://www.sl.universalservice.org/menu.asp Under Form 471, click the "Display" Enter your Form 471 application number (you don't need the security code) Click on Original to select Click the Display button. You can see certification status at the top. Click "Display Entire Application" button if you need a copy of the Form 471 for your files. If you haven't done your Item 21 Attachment ... Go to: http://www.sl.universalservice.org/menu.asp Under Form 471, click the "Item 21 Attachment" button Enter the entity number, application number and security code. Check your Form 471 Application Status Go to: http://www.sl.universalservice.org/menu.asp Under Form 471, click the "Application Status" Enter your entity number, then click the Search button. Scroll down and click the Continue button. Look to the far right and see what it says under Application Status.
