FORM 472 For the Funding Year July 1, 2007 - June 30, 2008 Schools and libraries should be filling out the Form 472. Some Form 472 tips and information: ? For most of you the deadline will be October 28, 2008. ? You must have paid your last bill. All services must be paid for before requesting reimbursement. We encourage you to use the online form when possible. Please check with your phone company to make sure they are processing the Form 472 (BEAR) online. Fairpoint, Verizon, AT&T all use the online system. Some smaller Maine companies have not yet integrated the online system. Oxford Networks still requires the Form 472 to be faxed. If you file online and your vendor doesn't use the online system your Form 472 won't get processed and you will lose your money FORM 486 For the Funding Year July 1, 2008 - June 30, 2009 Schools and libraries should be filling out the Form 486. Some Form 486 tips and information: ? For most of you the deadline will be October 29, 2008. ? If you received a Funding Commitment Decision Letter (FCDL) after July 1, the deadline will be 120 days after the date of your FCDL. This year those of you that had Verizon designated on your Form 471 will need to use the new Fairpoint SPIN number. You should have received a letter from Fairpoint earlier this month with the details. USAC/SLD made a Global SPIN change so even though your FCDLs say "Verizon" and have the Verizon SPIN the forms have been changed in USAC's database to Fairpoint. So to successfully submit the Form 486 you will need to use the new Fairpoint SPIN number (143032501). Important note for ATM and phone customers who were Verizon customers and have been changed to Fairpoint. ? The Form 472 should be submitted to Verizon and will be processed by Verizon for funding year July 1, 2007 - June 2008. ? This was an agreement reached between Fairpoint and Verizon. ? Fairpoint will process all Funding year 2008. School district staff responsible for filing MSLN E-rate paperwork should plan to attend an all day training session either in the Bangor area on November 5th or the Portland area on November 7th . These training sessions for school districts will be led by Linda Schatz, President of EdTech Strategies and formerly Vice President of the Schools and Libraries Division of USAC (the E-rate program administrator). Additional information as to location and start/end times will be provided shortly, but please save these dates for training. Note: These sessions will be for districts using NSLP data for E-rate discounts and will not include training for those who must use alternative discount mechanisms.
