According to the study, which also included Australia, Canada, France, Germany, the Netherlands, New Zealand, and the United Kingdom, U.S. patients are significantly more likely to call for fundamental change in their country's health care system, with one-third saying the system needs to be rebuilt completely. The journal Health Affairs published the findings today as a Web Exclusive. More than half (54%) of U.S. chronically ill patients did not get recommended care, fill prescriptions, or see a doctor when sick because of the cost, compared with 7 percent to 36 percent in the seven other countries. About one-third of U.S. patients--again, the highest proportion--experienced medical, medication, or lab/diagnostic test errors. One-third encountered poorly coordinated care, including duplication of tests or medical records that were unavailable at the time of an appointment. And, reflecting the pervasiveness of high cost-sharing and coverage gaps, 41 percent of U.S. patients spent more than $1,000 in the past year on out-of-pocket medical costs, compared with 4 percent in the U.K. and 8 percent in the Netherlands. The nation's second-biggest tobacco company said that dissolvable strips, orbs and sticks _ made from finely milled tobacco _ will be sold early next year, starting in Columbus, Ohio; Indianapolis; and Portland, Ore. They come in fresh and mellow flavors. Among their biggest selling points for smokers, who have fewer and fewer places to light up, is that there is no spitting and nothing left to throw away. Cigarette companies are trying to find new ways of selling tobacco as cigarette demand has fallen because of smoking bans, health concerns and social pressure. They are focusing more on cigars and smokeless products such as moist snuff, chewing tobacco and snus. Anti-tobacco groups objected last month when Reynolds first said it would begin selling dissolvable tobacco. "These new products pose serious threats to the nation's health," a statement from the Campaign for Tobacco-Free Kids said then. "They are likely to appeal to children because they are flavored and packaged like candy, are easy to conceal even in a classroom and carry the Camel brand that is already so popular with underage smokers." Reynolds, which sells Camel, Kool and American Spirit cigarettes, defended the new dissolvable tobacco in part by saying the products come in child-resistant packs. The company's dissolvable tobacco products come in three shapes. Orbs are pellets that last about 15 minutes; strips last five minutes or less; and sticks, which are shaped like a toothpick but slightly bigger, last about 15 to 20 minutes. They are likely to cost less than cigarettes, spokesman David Howard said. "We're very excited about the potential of these modern smoke-free products," Brice O'Brien, a senior vice president of growth and innovation, told investors at a conference on Monday. O'Brien said it was too early to tell if dissolvable tobacco would prove to be a breakthrough innovation, like menthol or filtered cigarettes. But he said focus group testing on adult smokers showed greater interest for dissolvable tobacco than for snus. Snus is teabag-like pouches that users stick between the cheek and gum. Reynolds, which first started selling snus in April 2006, plans to launch Camel Snus nationwide, also in the first quarter. Using a rigorous economic model developed to assess the cost-effectiveness of community-based physical activity interventions, the study found these interventions to be cost-effective; reducing new cases of many chronic diseases and improving quality of life. Researchers found that community-based physical activity programs appeared to reduce new cases of disease by: 5-15 cases per 100,000 people for colon cancer; 15-58 cases per 100,000 for breast cancer; 59-207 cases per 100,000 for type 2 diabetes, and 140-476 cases per 100,000 for heart disease. Community-based physical activity interventions broadly fall under the following strategies: * Community campaigns such as mass communication efforts (TV/radio, newspapers, billboards, advertisements). * Social support networks such as exercise groups to encourage behavior change. * Tailored behavior change to encourage people to set physical activity goals and monitor their individual progress. * Enhanced access to services that support active lifestyles such as fitness centers, bike paths and walking trails. "Our study found that public health strategies that promote physical activity are cost effective, and compared with other well-accepted prevention strategies, such as treatment for high cholesterol or motor vehicle air bags, offer good value for the money spent," said Larissa Roux, M.D., Ph.D., lead author of the study. The study, "Cost Effectiveness of Community-Based Physical Activity Intervention," is being published in the online version of the American Journal of Preventive Medicine. "This study supports the value and effectiveness of the physical activity interventions that were studied," said William Dietz, M.D., Ph.D., director of CDC's Division of Nutrition, Physical Activity and Obesity. "This study also shows the importance of the new physical activity guidelines put forth last month by the U.S. Department of Health and Human Services.'' The HHS guidelines recommend: * Two and a half hours each week of moderate-intensity aerobic activity, such as brisk walking or, * An hour and 15 minutes each week of vigorous-intensity aerobic activity such as jogging or running. In addition, all adults should include muscle strengthening activities that work all the major muscle groups on two or more days per week.
