There is funding attached to the SCHIP that is heading to Obama's desk - $25 million in demonstration projects re: childhood obesity. We are pleased to share the news that, in its current state, the bill includes a section for childhood obesity demonstrations. A summary is below. SCHIP's Childhood Obesity Demonstration Project Overview of Bill Content Medicare/Medicaid Services, through funding from SCHIP, will be awarding $25M over five years for demonstration projects that aim to effectively reduce childhood obesity. Grants will be awarded to local municipalities, educational agencies, universities, and colleges, health centers, health care providers, and community based organizations. Priority for funds will be granted based on an organization or government's previous long-term success in fighting obesity, its ability to raise non-federal dollars, the content of its plan along with its strategy, and based on a program's alignment with the Healthy People 2010 Plan of the State. Priority will also be given to impoverished and underserved communities and that encourage collaboration and integration of a number of different stake-holders. Grantees will be able to use the federal funds to: identify risk factors, develop screening tools, and implement age-appropriate interventions in the areas of prevention and treatment. . There is also a strong community component to the grant. Grants will be awarded to provide community education, help community health workers educate families and guide parents regarding healthy eating habits, exercise, and modeling positive behavior. Although the details are forthcoming, I thought I would share with you what I do know. Governor Baldacci has set up a website (http://www.maine.gov/governor/baldacci/policy/Recovery2009.shtml ) so that anyone can find information on the Stimulus Package and its impact on and opportunities in Maine. I encourage you to check it out and to monitor it for information. In terms of public health, we expect three major categories of funding: Drinking Water, WIC, and Wellness and Prevention funds. A short summary of each is below. Drinking Water - The Stimulus Package includes $2 billion through US EPA to address drinking water infrastructure needs in order to protect public health. From these funds, we expect Maine will receive ~$19.5 million for drinking water construction projects. Maine CDC's Drinking Water Program staff are working very hard, gearing up to move these funds quickly so that they can be used for this summer's construction season. We expect these funds will provide a number of jobs across the state in addition to providing the ability for communities to expand economically since most of these projects are helping communities to meet federal EPA Clean Drinking Water standards to protect public health and/or build capacity in an overburdened system. Accomplishing these objectives means a community can then add customers, which helps our economy. These funds are in addition to about $8 million in state bond and federal EPA funds for this year, bringing the total to $27.5 million for drinking water projects in 2009. Because Maine's drinking water systems are some of the oldest in the country (Maine CDC's predecessor, the Maine Board of Health, received the authority to regulate drinking water in 1903), we have a large number of communities waiting for loans and grants to complete needed upgrades to their water systems. There are about $100 million in such projects, so the over $27 million for 2009 will help meet the backlog in demand. FMI about Drinking Water Program: http://www.maine.gov/dhhs/eng/water/ WIC - We know the Stimulus Package appropriates $500 million for WIC nationally, some of this for the development of information management systems and some to meet the food demands as a result of higher numbers of WIC clients because of the poor economy. We do not know how much Maine will be receiving from these funds and we do not know the break down of how we will be expected to spend them. We know that the Maine WIC is running a deficit because of high numbers of WIC clients (~27,000, relative to 18,000 earlier this decade), and last year the federal government provided some supplemental funds so we did not have to cap enrollment or de-enroll people (WIC is not an entitlement program). We are hoping that there are sufficient funds to meet our deficit this year so we can continue to serve low income women and young children in Maine and to complete the required upgrade of our information management system. WIC provides supplemental food, nutrition education, and some health screenings to low income (185% of federal poverty level) pregnant women, women with young children, and young children (up to age 5). FMI about WIC: http://www.maine.gov/dhhs/wic/ Wellness and Prevention Funds - We know there is $1 billion in this category nationally, but with few specifics. The breakdown of these funds is: $400 million for vaccines (the Title 317 funding category), and we believe the focus of this may be on childhood influenza vaccine. $50 million for states to implement strategies to reduce health care associated infection rates. $650 million for evidence-based chronic disease prevention strategies using the US CDC's Community Preventive Services as the guide for how these funds are spent (http://www.thecommunityguide.org/index.html ). THIS INFORMATION IS CURRENT AS OF NOON FRIDAY FEBRUARY 27. This information is based on a meeting I and other public health leaders had with Dora Hughes MD, MPH, and regular contact by me, ASTHO staff and Mark Mioduski with public health, administration and congressional staff leaders in Washington and Atlanta. We strenuously argued for money to flow to the CDC and through state public health agencies. Points: * It is unknown how much of the $650 Million of Prevention and Wellness (P&W) will go to state health departments. * No definitive models or funding mechanisms have been adopted for how the $650 Million for Prevention and Wellness will be used by the HHS Secretary. * These dollars may not move as quickly as stimulus dollars in other areas, so there can be more thought on how they can be a down payment on health reform * The White House and Dora Hughes are formulating plans and looking for a large initiative integrated across HHS. * HHS may hold back $550 million to invest in the "big idea." CDC may or may not get to manage the dollars for the big idea. * Dr. Besser is chairing the internal HHS stimulus workgroup on the P&W dollars. The workgroup is reviewing over 80 proposals from across HHS for P&W activities seeking funding * It is unclear how much funding will be passed to CDC. Rumor has it that CDC would get $100 M for Healthy Communities and perhaps another $1M for a few other areas yet to be determined * Congress provided $300M for the Sec 317 Immunization Program in the stimulus package. ASTHO is participating in a working group with CDC and states to provide input on the funding allocation formula and reporting requirements. Additionally, many states have referred to data from FFIS on state allocations of 317 stimulus funding. CDC's Immunization Program informed ASTHO that these data are not accurate, as allocations have not yet been determined. We urge caution in using these allocations to make plans for spending the immunization dollars. * The stimulus package included $50M for states to address Healthcare-Associated Infections. ASTHO is talking with HHS and CDC officials about options, and is planning to convene a conference call for states to provide input to CDC. PUBLIC HEALTH AND COMMUNITY PREVENTION WILL BE JUDGED BY HOW SUCCESSFULLY WE MOVE THE MONEY WE RECEIVE AND HOW WE DEMONSTRATE 2 THINGS: 1. Health Impact on a national scale 2.
