would be pertinent to your prevention colleagues please forward that

Dear Customer:

 

The Office of Substance Abuse Access Change Team is conducting a brief
survey to assess efficiency and address any barriers in accessing our
staff.  It is important to the staff at the Office of Substance Abuse
that our customer's questions, concerns, and issues are addressed in a
timely fashion.  One way to address this is to gather data on where and
how our customers are connecting with us as well as data on the response
time and the quality of the response. Once the survey closes, we will
compile that data and inform you of the outcomes and changes as a result
of your input.

 

If you would, please take a moment to complete the survey.  We will be
closing the survey on December 1, 2008.
