[Provider] Servicing Providers on Paper MaineCare Claims

Thank you to those of you who reminded us that this is in regard to paper
claims until January 19 when it also applies to electronic claims.

Please remember that when submitting paper claims to MaineCare, servicing
provider
IDs must be included for each line item on the HCFA 1500 claim form and the
dental claim form.   New ID numbers should now be used.
