[Provider] UB04 Claim Forms Accepted By MaineCare

ATTENTION:  All MaineCare Providers Filing Institutional Claim Forms

 

The Office of MaineCare Services now accepts the updated UB04 claim
forms.  Billing instructions for filing these forms with MaineCare can
be found at
http://www.maine.gov/dhhs/bms/providerfiles/provider_billing_manuals.htm

 

These are the instructions for filing UB04 claims on paper.  Electronic
submissions of these claim forms are not accepted.

 

In addition, the UB92 claim forms will continue to be accepted until
further notice.
